The granulomatous cutaneous disease, or pyogranulomatous sterile idiopathic, is rare in dogs, but much more rare in cats. Was attended in Veterinary Center Dr. Leonardo Torres, in Patos, PB, a male dog, race Cocker Spaniel, 13 years old, with 12 kg of body weight and main clinical complaint of nodules in the palm and plant pads and suppurative injuries in the snout, this containing a serosanguineous drainage tract, and with dry and rough coat with history of butthead growth. At the clinical examination, the animal has presented apathetic, tragic faces and presence of swellings in dorsal cervical region. Were collected samples of blood to the Hemogram and hormonal dosage of Total Triiodothyronine (T3) and Total Thyroxine (T4). In Hemogram was observed anemia with normochromia and normocytosis. The leukogram showed a leukopenia with an increase of segmented, lymphopenia and thrombocytopenia. By the suspect of Ehrlichiosis was established a therapy with Pyrimethamine (5 mg/kg oral /12/12hours, 21 days). Both of the dosages of T3 and T4 were below of the normal values reference (77.53 nmol/L and 2.22 μg/dL, respectively) being established the therapy with Levothyroxine (0.04 mg/kg oral 12/12 hours). The microbiological culture requested from secretion of the wound of the snout identified Staphylococcus coagulase, with microbial sensitivity to Ciprofloxacin (5 mg/kg oral, 12/12 hours during seven days), but was not successful. Then, was performed a surgical excision of the nodules of snout and left palm pad, being the removed material fixed in formalin 10%, and sent to histopathologic evaluation. Was observed epidermis with moderate acanthosis, irregular and epidermolytic ortoquetotic. In the dermis, nodular and diffuse injuries, composed of severe inflammatory infiltrate neutrophils, macrophages, lymphocytes and plasma cells. The follicles near to the inflammatory process were expanded and completed by keratin and absence of neoplastic cells. Clinically was revealed a sterile/idiopathic immunomediated pyogranuloma. Was established a treatment with Prednisolone (4mg/kg) 12/12 hours during 15 days and reduction of the dose to 2 mg/kg for more 15 days, getting the end of clinical signs with 25 days of treatment. It is concluded that the use of additional tests in diagnosis of skin diseases in the medical clinic of small animals is essential for adoption of efficient therapeutic protocols front to non-specific changes. The therapy with systemic glucocorticoid proved efficient control of pyogranuloma, but the influence of Hypothyroidism on the appearance of this immunomediated dermatopathy was little clarified.
