Application of New Modified Fascia Lata Overlap in 37 Cases of Small Breed Dogs with Patellar Luxation

Canine patellar luxation is a common orthopaedic problem in toy and small breeds and various surgical techniques have been described. This study aimed at assessing the 1 year effects of a new modified fascia lata overlap in 37 cases. The modified fascia lata overlap and wedge recession trochleoplasty were applied to 20 dogs in total, 37 cases of 2~4 kg dogs having been diagnosed as patellar luxation grade 1~3 for the last 2 years, after receiving the agreement of the owner. We thought that medial and lateral femoropatellar ligaments had a role in interference in the depressing of the patellar into the groove. Even sufficiency of the trochlear depth is established by trochleoplasty. In the modified fascia lata overlap, both medial and lateral femoropatellar ligaments were incised proximal to the patellar. The patellar sits centered in the groove while suturing both incised ligaments directly. Operation time and reluxation were compared with those of 38 cases, which were done by traditional fascia lata overlap, wedge recession trochleoplasty and tuberosity transposition methods. In 4 cases having been diagnosed with grade 1, applying modified fascia lata overlap solely showed stabilized joint, and reluxation didn't occur. In 8 of 9 cases diagnosed with grade 2, applying modified fascia lata overlap without trochleoplasty showed reluxation in a few weeks. But in 32 cases of grade 2 and 3, applying a modified fascia lata overlap and wedge recession trochleoplasty together did not show any reluxation after 1 year. The mean operation time was 17 minutes performing the modified fascia lata overlap and wedge recession trochleoplasty together. Most of the compared cases also showed good results, but the mean operation time was 27 minutes in performing the fascia lata overlap, wedge recession trochleoplasty and tibial tuberosity transposition. We conclude that the new modified fascia lata overlap with and/or without trochleoplasty is a simple surgical method and shows good stabilizing results through the excellent depressing effect of the patellar on the groove in grade 1-3.
